Shingles Vaccine Availability & Wait List Form

Why Get Vaccinated? SHINGLES, also called zoster or herpes zoster, is a painful skin rash, often with blisters caused by the varicella zoster virus, the same virus that causes the chicken pox.  After having the chickenpox, the virus says in your body and can cause shingles later in life.

Shingles is not contagious. However, a person that has never had chickenpox or the chickenpox vaccine may get chicken pox from someone with shingles.

SHINGRIX Vaccine was approved by the FDA in 2017 and was more than 90% effective in preventing shingles and also reduced the likelihood of long lasting pain with the rash.  It is recommended that two doses-two to six months apart-be administered for effectiveness.

The WAYLAND HEALTH DEPARTMENT SHINGREX PROGRAM will acquire small batches of vaccine to be made available during 2019, to residents of Wayland, aged 65 and older.  The Health Department will be paying for one dose.  Because the SHINGRIX Vaccine is part of the Pharmacy benefit (unlike our FLU Program) there WILL BE resident out of pocket expense.  Residents will need to determine the cost effectiveness of receiving the vaccine from either the Health Department/their physician/or pharmacy.

  • HEALTH DEPT SAMPLE TEST:  If your pharmacy co pay for the Vaccine is greater than $130; then it is beneficial for you to put your name on the Health Department WAIT LIST.  We will be collecting your portion, which is equal to the cost of one dose, at your first appointment by cash or check made out to the Town of Wayland.
  • If your pharmacy co pay for the Vaccine is less than or equal to $130; then it is beneficial for you to go to your physician or pharmacy.

We ask each resident that is interested in obtaining BOTH doses of the SHINGRIX Vaccine through the Wayland Health Department to complete ONLY ONE - WAIT LIST SIGN UP FORM that can be found on-line here, at the Library, Council on Aging or Health Department.  These forms must be mailed back to the Health Department where you’ll be added to the WAIT LIST-(Note that duplicates will be discarded).  Being placed on the WAIT LIST is not a commitment from the Health Department to provide the vaccine as we will only be obtaining small quantities throughout the year.

Financial assistance may be available, watch our website for updates.

Once we have obtained the vaccine, names on the WAIT LIST will be randomly selected by a disinterested party. There is no need for you to call the Health Department as we will contact you to make appointments for both doses once your name has been selected.
