What forms do I need to fill out?

  • After being in a car accident you should fill out these 3 forms:
    • Accident Report Request Form
      • If you need a copy of an accident report, from an accident that happened IN Wayland, download this form. Once filled out, it must be dropped off at the Wayland Police Department, (38 Cochituate Road, Wayland, MA 01778)
      • This form can also be obtained at the Wayland Police Department any time if needed.
    • Motor Vehicle Accident Information Form
      • This is a good form to have in your glove compartment, if you are in an accident this form reminds you to obtain the necessary driver and car information for your accident report form.
    • Motor Vehicle Crash Operator Form
      • If you were in a car accident, you are REQUIRED BY LAW TO SEND THIS FORM TO THE RMV & THE POLICE STATION IN THE TOWN THAT THE ACCIDENT OCCURED.

What to do immediatley after

  • After a car accident, the driver should call 911 to get a police officer on the scene, so they can write an accident report.
  • You should not move your vehicle under these situations:
    • If the vehicle has been totaled or not running properly.
    • If somone is injured.
    • If there is too much debris on the road due to the accident.
  • Most serious accidents usually require all vehicles being left in place after the car accident.
    • The police will need to recontruct the accident scene, and if the vehicles have been moved, it makes it much more difficult for the police to conduct their investigation.

The Aftermath

  • Call your car insurance company, and tell them you've been in an accident. Follow their guidance from there on.
  • The police on the scene will also give you information about how to move forward after.
