Camper Health Records

Parents: A child cannot attend summer camp without a completed camper health file as required by MDPH Camp Regulations; all records need to be on file by June 15. 

    • In April 2024, after you enroll in camp, you will receive an “invitation” email from campdoc.com at the same email address you provided at registration. Check your inboxes for emails that will allow you to complete/update your 2024 Camper Health Record and permission slips.  All Camper Health Records will be submitted through CampDoc.com. Some 'Welcome to CampDoc' emails may land in your spam/junk folder. 
    • We partner with campdoc.com to provide parents with an electronic health record system to submit your campers health information. The DocNetwork platform includes CampDoc and SchoolDoc, an electronic health record system to help camps, schools, and organizations manage health information electronically. If you need help setting up your account or submitting health forms, please call the Rec Office. 
    • The security and privacy of your campers information is important to us. The campdoc.com site is secure, encrypted, and password protected; only the necessary, authorized and trained Wayland Rec camp health care staff will have access to your campers health record.

    All Campers and Staff Health Records Require:

    1. Required Camper Health History 
    2. A recent photo of the camper
    3. Immunization record with date of last tetanus shot
    4. Evidence of physical exam within 18 months of camp (on of after December 23, 2022)
    5. Permission Slips for Swimming and Trips

      Medications, Epi-Pens, Inhalers:

      • Parent Medication Authorization for Staff to Administer Medication to a camper 
        • If campers will take Rx at camp
        • Required for ALL prescription and non-prescription medications, epi-pens and inhalers to be administers by camper or camp staff, during the camp day.
      • If a physician has indicated that a camper requires emergency medication, such as an inhaler or epi-pen, the camp MUST HAVE THE MEDICATION IN HAND along with a signed medication authorization form in order for the camper to attend. Unless otherwise contraindicated in writing.

      This camp complies with regulations of the Massachusetts Department of Public Health and be licensed by the local Board of Health. Parents of licensed camps may request copies of background checks, health care and discipline policies as well as procedures for filing grievances.105 CMR 430.000:  Minimum Standards for Recreational Camps for Children (State Sanitary Code, Chapter IV)
